'Marry' his adoptive father off to his birth mother, then make a note about the relationship. You can always add a duplicate of him so as to make him his birth mother's son as well as the adoptive son. ie John (1) as the son of the people who raised him, and John (2) as the natural son of his birth mother.

If you know the name of his birth father, you can add him as a 'spouse' to his birth mother. If not, just leave it as the default 'unknown'.

Someone is bound to say - it is not advisable to add the names of living people to your tree without their permission.
